Very Rare Magic Items in D&D 5e
The 5e SRD has 68 very rare magic items across the 2014 and 2024 rules, listed below by type. 37 of them require attunement. Under the 2024 SRD a very rare item is worth 40,000 GP (20,000 GP for a consumable). The most looked-up are Potion of Healing, Staff of Power, Ioun Stone and Belt of Giant Strength.
Very rare items are high-level treasure: +3 weapons, +2 armor, the Staff of Power. The 2024 SRD says items this rare might be sold only in wondrous places, such as a city on another plane of existence.
Rows with a 2024 or 2014 badge are very rare in that edition only: a new item, or one whose rarity changed. Unbadged rows are very rare in both.

Buying, selling and crafting very rare items
The 2024 SRD prices a very rare item at 40,000 GP. Halve that for a consumable other than a Spell Scroll, and add the base item's cost for armor and weapons. Crafting one takes 125 days of work and 20,000 GP in raw materials, also halved for consumables; the crafter needs Arcana proficiency and the right tool. The 2014 SRD has no prices or crafting rules for magic items, so 2014 tables usually follow the Dungeon Master's Guide or a house rule.
