    A Necklace of Adaptation 5e is an uncommon wondrous item that requires attunement and lets you breathe normally in any environment: underwater, in smoke, in an airless vault. Its second benefit changed between editions. In 2014 it gives advantage on saves against harmful gases and vapors such as Cloudkill, Stinking Cloud and some dragon breath; in 2024 it gives Advantage on saves to avoid or end the Poisoned condition. The verdict: a strong pick for an underwater or planar campaign, and a weak use of an attunement slot anywhere else.

    The breathing sentence is identical. The second half went from a list of hazards (gases and vapors) to a condition (Poisoned), and those two cover different threats.

    How the Necklace of Adaptation Works

    Breathing normally in any environment

    The text has no exceptions. Underwater, in thick smoke, at the bottom of a flooded shaft or inside a sealed room with no air, the wearer breathes as usual, so the suffocation rules never start. That includes the air limit inside a Bag of Holding: a wearer stuffed into one doesn't run out.

    Breathing isn't the same as moving or fighting. Underwater, the SRD's underwater combat rules still apply to a wearer without a Swim Speed: Disadvantage on most melee weapon attacks (2014: unless the weapon is a dagger, javelin, shortsword, spear or trident; 2024: unless it deals Piercing damage), and ranged weapon attacks miss beyond normal range and have Disadvantage within it (2014 exempts crossbows, nets and thrown weapons like javelins). A Ring of Swimming (Swim Speed 40 feet) covers the movement half.

    2014: advantage against harmful gases and vapors

    The 2014 necklace names its examples: Cloudkill, Stinking Cloud, inhaled poisons, and the breath weapons of some dragons. The save gets advantage whatever the effect does, damage or condition.

    • •Cloudkill (2014): Constitution save against 5d8 poison damage. The spell affects creatures even if they hold their breath, so the necklace's breathing does nothing here, but the advantage applies.
    • •Stinking Cloud (2014): creatures that don't need to breathe automatically succeed. The wearer still breathes, so they don't get the automatic success; they roll with advantage.
    • •Inhaled poisons: the SRD says holding your breath doesn't help against them. Advantage does.
    • •Dragon breath: "some dragons" isn't defined. A green dragon's Poison Breath is a poisonous gas by its own description, so it qualifies. DM ruling: fire, lightning, cold and acid breath don't count as gases or vapors; a breath that's described as gas does.

    2024: Advantage against the Poisoned condition

    The 2024 necklace helps whenever a save is made to avoid or end Poisoned, from any source, gas or not. It doesn't help against Poison damage.

    What Advantage is worth: against a DC 16 Constitution save with a +2 bonus, you succeed on 14+ (35%) normally and about 58% with Advantage.

    Attunement

    Both editions require attunement, by anyone. The breathing benefit needs attunement too, so a necklace handed to an unattuned ally at the bottom of a lake does nothing until they spend a short rest attuning.

    At the table: the 2014 necklace protects against area spells and breath weapons, which hit hard. The 2024 necklace protects against a condition, which comes up more often but does less damage. Against a party facing a green dragon, the 2014 version is much better; against ghasts and hezrous, the 2024 one is.

    Is the Necklace of Adaptation Worth It?

    In the right campaign, yes. An adventure under the sea, on the elemental planes, or in a city of poisonous smog makes the breathing benefit permanent and party-saving. The alternatives are temporary: a Potion of Water Breathing lasts 1 hour (2014) or 24 hours (2024), and the Water Breathing spell lasts 24 hours and costs a 3rd-level slot.

    Elsewhere, compare it with the other uncommon attunement items. In 2024, a Periapt of Health gives the same Advantage against Poisoned plus a daily 2d4 + 2 Hit Points of healing, with no breathing. A Periapt of Proof against Poison (rare) gives immunity to Poison damage and the Poisoned condition outright. Of the three, only the necklace handles air.

    When it's a trap: in a land-based campaign with few poisonous creatures, where it sits in an attunement slot that a Cloak of Protection would use better.

    Necklace of Adaptation for DMs

    Where it turns up. Just before the party needs it: the treasure in the dungeon above the flooded level, or the reward from a sea-going patron. Hand out one, and the party has to decide who goes in the water.

    What it changes. Water stops being a barrier for one character. Smoke from a burning building and the stale air of a sealed tomb both become manageable. Plan underwater sections around the wearer scouting alone.

    Pricing. The 2024 SRD values uncommon items at 400 GP. The 2014 SRD has no prices. Use the magic item rarity calculator if you need a 2014 figure for a shop.

    Hooks.

    • •A pearl diver's family heirloom lets her stay down for hours. She'll lend it to the party for one dive, and she wants half of what they bring up.
    • •A sealed vault has no air left inside. The necklace is in there, on the skeleton of the last thief who tried.
    • •An alchemist's tower leaks poison smoke into the street, and the only person who can walk inside is the one wearing the necklace.

    Frequently Asked Questions

    Does the Necklace of Adaptation let you breathe underwater?

    Yes. You breathe normally in any environment, including underwater, in both editions. It doesn't give a Swim Speed, so underwater combat penalties still apply.

    Does the Necklace of Adaptation require attunement?

    Yes, in both the 2014 and 2024 rules, by any creature. Its benefits only work while you're attuned.

    Does the Necklace of Adaptation protect against Cloudkill?

    In 2014 you have advantage on the save. In 2024 it doesn't help, because Cloudkill deals Poison damage and doesn't impose the Poisoned condition.

    Does the Necklace of Adaptation give poison resistance?

    No. It gives no resistance to Poison damage in either edition. The 2024 version gives Advantage on saves to avoid or end the Poisoned condition.
